Cancel service with carrier form
When cancelling service with a previous carrier please follow the following procedures.
- Carefully check all services are switched as once the account is closed you will lose any numbers left behind
- Generally 1-5 days after the cut date has taken place cancel the old carrier. Make sure you have the following information.
- Account number
- Person available who can authorize changes.
- Contact the previous carrier billing department to place an order to cancel out services.
- Make sure you get a reference number and write it down somewhere safe.
Complete the following form, once you hit submit the information will be emailed to you and your contacts then we suggest printing the screen.
